The double click protection is broken as well as much of the CSS with the latest wordpress. Looks like the author isn't providing support any longer as the recent posts go unanswered. Sucks, looks like it would be a great plugin.

Just what I needed for my site! Took a few minutes to setup my form as I had to create some custom fields, but it was pretty easy.

This plugin is just what I was looking for, answers all the needs I have, and does a great job, no bugs detected so far.
